1. Introduction
Welcome to NexGO LLC ( "NexGO, "we", "our", or "us" ). These Terms and Conditions govern your use of our transportation coordination services. 2. Use of Services
NexGO LLC provides non-emergency transportation coordination services for individuals with disabilities, including services funded through Medicaid and the Division of Developmental Disabilities (DDD). Services may include arranging transportation through third party providers and/or direct transportation services.
All services are non- emergency and are not a substitute for medical transportation or emergency care.
3. Payment Terms
For self-directed Medicaid/DDD participants, payment may be processed through a Fiscal Intermediary such as Public Partnerships LLC.

NexGO LLC will submit required documentation for services rendered

Payments are subject to approval by the Fiscal Intermediary and Medicaid program rules

NexGO LLC is not responsible for delays caused by third-party processing systems
Clients are responsible for services not covered by Medicaid or exceeding approved budgets.
NexGO LLC complies with all applicable Medicaid and DDD program requirements under the New Jersey Division of Developmental Disabilities.
For Medicaid/DDD-funded services:

Services must be authorized in the participant's Individual Service Plan (ISP)

Services must be medically necessary and within approved service limits.

NexGO LLC will only bill for services actually rendered and properly documented

All services are subject to state audits, verification, and compliance reviews
NexGO LLC agrees to comply with all applicable federal and state Medicaid regulations, including but not limited to those enforced by the Centers for Medicare & Medicaid Services.

NexGO LLC complies with applicable privacy laws, including the Health Insurance Portability and Accountability Act.
We protect all personal and health-related information and only use it for service coordination, billing, and compliance purposes.
6. Limitation of Liability
NexGO LLC provides non-medical transportation coordination only.
To the fullest extent permitted by law.

NexGO LLC is not liable for indirect or consequential damages

We are not responsible for third-party provider incidents

liability is limited to the amount paid for services rendered
7. Governing Law
These terms and any disputes arising under them shall be governed by and construed in accordance with the laws of the State of New Jersey
8. Prohibited Conduct
NexGO LLC strictly prohibits fraud, waste , and abuse in any Medicaid-funded service.
This includes but is not limited to:

Billing for services not provided

Misrepresenting service times or mileage

Submitting false documentation
Suspected violations may be reported to appropriate authorities, including New Jersey Office of the State Comptroller or Medicaid fraud units
9. Cancellation & No-Shows

Clients must provide at least a 1 hour notice for cancellations

No-shows or repeated late cancellations may result in service limitations

Medicaid may not reimburse for certain missed services
To comply with Medicaid/ DDD requirements NexGO LLC will maintain accurate and complete records including:

Trip logs (date, time, mileage , pickup/drop-off)

Service verification
All records will be retained in accordance with applicable laws and may be reviewed by state or deferral agencies upon request.

NexGO LLC may coordinate services through third party providers such as Uber/ Lyft
While NexGO LLC works to ensure quality and reliability, we do not control third-party providers and are not liable for:

Driver conduct

Delays or cancellations

Incidents occurring during transport
NexGO LLC supports and protects all participant rights under Medicaid and DDD Programs, including:

The right to dignity and respect

The right to safe and appropriate services

The right to choose providers
-
The right to file complaints without retaliation

NexGO LLC reserves the right to update these Terms at any time. Continued use of services constitutes acceptance of any changes.
